BEFORE YOUR APPOINTMENT:
If this is your first visit to Radiant Sun Acupuncture since June 1, 2020, please take a moment to review our consent form. You will be asked to sign it before your treatment, agreeing that we have taken all reasonable precautions to ensure your health and safety. If possible, please print and sign the form before your appointment.
Click here to download our COVID-19 consent form for treatment.
Note: If you have already signed a consent form with us, you do not need to do it again.
Please call before your appointment if:
You are experiencing any COVID-19 symptoms, including a temperature of 100.4 degrees or higher, cough, shortness of breath or difficulty breathing, fever, chills, loss of taste or smell, sore throat, headache, muscle pain or body aches.
You have had recent contact with someone diagnosed with COVID-19 or experiencing any of the symptoms listed above.
You have traveled internationally in the past two weeks.
What to expect when you arrive:
Masks are required and must fully cover your mouth and nose at all times. If you do not have a mask, one will be provided for you.
You will be screened either in the foyer or in the treatment room. This includes taking your temperature, checking your oxygen level and answering a few questions about any potential symptoms. If any COVID-19 symptoms are identified during your screening, you will not be admitted into the clinic and will be referred to seek further medical attention.
Please come alone unless you require physical assistance. You and anyone who accompanies you must be screened upon entering the building.
What we’re doing to keep you healthy:
Extra cleaning and sanitizing, including changing and laundering sheets after every patient and disinfecting common surfaces.
Wearing a mask and other protective clothing.
Limiting or eliminating use of communal objects (pens, magazines, beverages) and areas (waiting room, bathroom).
